Reduced Performance

q       Takes hours to do 20 minutes of homework?

q       Doesn't finish work in allotted class time?

q       Skips words and/or lines while reading?

q       Must re-read things to comprehend?

q       Falls asleep while reading?

q       Frequent after-school headaches?

q       Rubs eyes after reading for a short time?

q       Poor posture while reading (slumps)?

q       Holds books excessively close?

q       Doesn't work up to potential?

q       In math, does better with number problems than word problems?

q       Reverses words or letters?

q       Covers one eye with hand while reading?

q       Must use finger to follow when reading?

q       Reverses letters (b for d, was for saw)?

q       Often skips over short words?

q       Says wrong word when reading aloud?

q       Learns spelling words well and then cannot recall words during spelling test?

Avoider

q       Daydreams, especially at the end of the day?

q       Has trouble paying attention when it's time to read or do other book/close-up work?

q       Cannot sit still during quiet class times?

q       Can concentrate only for a short time when the subject involves reading?

q       Learns quickly by listening (auditory)?

q       Avoids work involving reading or other prolonged close-up activities?

q       More than 1 year behind in reading?

q       More than 2 years behind in reading?

q       Is bright in everything but academics?

q       Behavior problems?

q       Class clown, distracting others?

q       Moody, depressed about school and life?

Physical Changes

q       Often "buries" head in book, reading at excessively close distances?

q       When child looks up from book, they squint or blink?

q       Sometimes squints when looking at trees, or other distant objects?

q       Takes a long time to copy anything from chalkboard?

q       Family places high value on reading, studying, and academics?

q       Likes to sit very near to the television?

q       Peering behavior (Jaw thrust forward)?

q       Family history of vision problems?

q       Nearsightedness?

q       Astigmatism?

q       One eye stronger than the other?

q       Eye turn (in or out)?

q       Amblyopia (lazy eye)?

q       Already wearing glasses?

q       Glasses prescription has gotten stronger?
